29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:     a) Press operator - an employee was exposed to continuous noise levels at 173% of the allowable 8-hr time-weighted average level (TWA) or an equivalent level of approximately 93.9dBA. The sampling was conducted for 384 minutes during one shift on June 2, 2014. Zero exposure is assumed for the unsampled time of 96 minutes.  b) Press operator - an employee was exposed to continuous noise levels at 195% of the allowable 8-hr time-weighted average level (TWA) or an equivalent level of approximately 94.8dBA. The sampling was conducted for 366 minutes during one shift on June 2, 2014. Zero exposure is assumed for the unsampled time of 114 minutes.  c) Shear operator - an employee was exposed to continuous noise levels at 102% of the allowable 8-hr time-weighted average level (TWA) or an equivalent level of approximately 90.1dBA. The sampling was conducted for 377 minutes during one shift on June 2, 2014. Zero exposure is assumed for the unsampled time of 103 minutes.
