29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):      a)  Work Site:  115 Greenwood Avenue, Madison, NJ     Employee engaged in residential roofing activities approximately 32 feet from the ground below, with a roof pitch of 12 X 12, was not utilizing any type of fall protection, thereby exposing the employee to fall hazards.         Violation observed on or about 4/28/14.    GUARACA TORRES HOME IMPROVEMENT WAS PREVIOUSLY CITED FOR A SERIOUS REPEAT VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(b)(13) which was contained in OSHA INSPECTION 315639732, CITATION 1, ITEM #2, ISSUED ON 12/26/2011 WITH RESPECT TO A WORK PLACE LOCATED AT 10 WHITE OAKS ROAD, SAYREVILLE, NJ 08872 AND BECAME A FINAL ORDER ON 7/23/2012.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.      a)  Work Site:  115 Greenwood Avenue, Madison, NJ    Extension ladder was not extended at least three feet above the working surface of the roof to allow safe access on and off the roof.         Violation observed on or about 4/28/14.    GUARACA TORRES HOME IMPROVEMENT WAS PREVIOUSLY CITED FOR A SERIOUS REPEAT VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD OF 29 CFR 1926.1053(b)(1) WHICH WAS CONTAINED IN OSHA INSPECTION 315639732 CITATION 1, ITEM 003(b), ISSUED ON 12/26/2011 WITH RESPECT TO A WORK PLACE LOCATED AT 10 WHITE OAKS ROAD, SAYREVILLE, NJ 08872 AND BECAME A FINAL ORDER ON 7/23/2012.
