29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:    a. 960-982 Lake Ave, Clark, NJ - Stone Hill Village Development    An employee engaged in residential roof installation activities was observed climbing an aluminum extension ladder while carrying the materials lift to the edge of the roof.  The employee did not maintain at least one hand on the ladder at he progressed up and down the ladder.     Violation observed on or about July 31, 2015

29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:  a. 960-982 Lake Ave, Clark, NJ - Stone Hill Village Development  Employees engaged in residential roof installation were observed climbing an aluminum extension ladder while carrying the materials lift to the edge of the roof.  The employee climbed the extension ladder with one end of a materials lift (ladder section) on his shoulder as a second employee held the base which could cause the employee to lose balance and fall. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. 960-982 Lake Ave, Clark, NJ - Stone Hill Village Development    Employees engaged in residential roof installation were observed working from a second story roof without the use of fall protection, safety nets systems, or a guardrail system.  Employees worked on a 6/12 pitch roof at the eaves without the use of fall protection.      Violation observed on or about July 31, 2015      Guaraca Torres Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 979076, citation number 1 item number 1, and was affirmed as a final order on July 14, 2014, with respect to a workplace located at 115 Greenwood Ave, Madison, NJ.     Guaraca Torres Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315639732, citation number 1 item number 002, and was affirmed as a final order on May 2, 2012, with respect to a workplace located at 10 White Oaks Road, Sayreville, NJ 08872    Guaraca Torres Home Improvement was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313982860, citation number 1 item number 003, and was affirmed as a final order on May 27, 2011, with respect to a workplace located at 477 South Pine Ave, South Amboy, NJ 08879.
