29 CFR 1910.178(l)(1)(i): Operators were not trained in the safe operation of powered industrial trucks: (a)Tursios Roofing @ 4363 South Quebec Street, Denver, CO 80237: On and before 5/28/2014, the employer did not ensure all forklift operators were trained in operation of powered industrial trucks in that the operators were not trained in the use of the JCB rough terrain forklift prior to use. This condition exposed the employees to the hazard of improper forklift operation. Abatement Note: 29 CFR 1926.602(d): Powered industrial truck operator training. The requirements applicable to construction work under this paragraph are identical to those set forth at 29 CFR 1910.178(l) of this chapter. Abatement Note: Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems. (a)Tursios Roofing @ 4363 South Quebec Street, Denver, CO 80237: On and before April 5th, 2011, the employer did not ensure that fall protection systems were utilized by employees engaged in roofing activities while working at the unprotected edges of a roof on a multi-family home. Employees were not utilizing a fall protection system. Employees were exposed to fall hazards of approximately 30 feet.
