OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: A.) Sears Auto Center #6254, New Castle, PAService Bay AreaAuto Lifts: On or about June 2, 2014 and at times prior there to, employees were potentially exposed to struck-by hazards while working under and/or near potentially defective automotive lifts due to a lack of monthly automotive lift inspections. The monthly inspections were not being completed for the auto lifts. Potential automotive lift deficiencies included but were not limited to excessive wear/scouring to the track on the vertical column for Rotary Lift #6.

Sears Auto Center #6254, New Castle, PATire Changing Machine: On or about June 2, 2014, the flexible cord for the tire changing machine was frayed and damaged. The outer jacket of the cord was pulling away from the cord cap exposing the internal conductors.

Sears Auto Center #6254, New Castle, PA On or about June 2, 2014, electrical panel box located on the back rear side of the facility near the tire room contained circuit breakers that were not labeled.
