The trench is approximately 45 feet long and varies in depths from 4 feet at the ends and 6 feet in the center. The employer failed to provide trench boxes, hydraulic shoring or implement another approved means of protection such but not limited to sloping or benching of the soil.

Two, eight foot fiberglass step ladders were being used inside of a trench for egress during the inspection.     ANSI- A14.5-2007: 9.3.1.2- states that self-supporting ladders shall not be used as single ladders or in the partially closed position.
