29 CFR 1926.451(a)(1): Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: a/ At the job site, one employee was anchoring structural steel components while standing on the top rail attached to a tubular weld, mobile scaffold. The fall distance to the concrete pad was approximately 15 feet.

29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: a/ At the job site one employee was connecting structural steel components together while standing on the top rail attached to a tubular weld, mobile scaffold. All four casters were not locked to prevent movement.
