29 CFR 1903.19(d)(1): The employer did not submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a)    The employer B & W Masonry Corporation failed to provide documentation demonstrating the abatement methods and completion as cited in Citation 1 for hazards associated with: 1-1)  employee(s) exposed to fall hazard greater than 12 feet while performing masonry work on a Bill Jax Tubular Welded Frame Scaffold platform that was not fully planked; and 1-2)  Employee(s) performing masonry work from a Bill Jax Tubular Welded Frame Scaffold were not trained to recognize hazards associated with the type of scaffold being used.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  Location: 6355 Harrison Ave, Cincinnati, OH. On or about 6/03/14 an employee was exposed to a fall hazard greater than 12 feet above the ground below while performing masonry work on a Bill Jax Tubular Welded Frame Scaffold platform that was not fully planked.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Location: 6355 Harrison Ave, Cincinnati, OH. On or about 6/03/2014 an employee working for B & W Masonry Corporation performing masonry work from a tubular welded frame scaffold had not receive training to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.
