29 CFR 1926.100(a): Employee(s) working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:  a/At the job site, one employee was working from a step ladder while a second employee worked from an approximate 12 ft. high ladder jack scaffold directly above.  The employee working from the ladder jack scaffold was using a tool.  The employee working from the step ladder was not wearing a hard hat.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toe boards, screens, or guardrail systems, through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects, or equivalent.  a/  At the job site, a 10-gallon plastic bucket was positioned on the approximately 12 inch wide pick board belonging to a ladder jack scaffold.  Another employee was working from a step ladder directly below.  Neither a canopy nor any other form of equivalent falling object protection was provided.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:   a/ At the job site, the two brackets supporting the pick board belonging to a ladder jack scaffold were pointing outward.  Safe access in the form of a third portable extension ladder was not in use.
