29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used: Employees are exposed to electrical shock hazards from an extension cord that had been repaired and the employer did not ensure that the repair splice retained that the outer sheath properties.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Employees removing part of the second (2nd) floor were exposed to fall hazards and the employer did not ensure that appropriate fall protection was utilized.
