29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: 1) Production Area - The employer failed to provide refresher training and evaluation for an employee that was involved in a forklift accident. On May 30, 2014, an employee was operating a forklift truck when he struck a pallet that hit an employee causing her to fall and sprain her wrist.
