Such activities shall be performed only by experienced and trained employees selected for such work by the competent person:       In the water retaining pit, Northeast, on or about June 2, 2014 and at times prior thereto, employees were exposed to a fall hazard while dismantling a scaffold without the supervisors direction.

All protruding reinforcing steel, onto and into which employees could fall, was not guarded to eliminate the hazard of impalement:    In the water retaining pit, Northeast, on or about June 2, 2014 and at times prior thereto, employees were exposed to an impalement hazard while working from a scaffold that had 392 exposed rebar on the perimeter of the pit walls.
