In the pallet recovery shop, while operating  the single end power feed trim saw # 1, employees were exposed to a caught in hazard, in that, the saw pulley was not guarded, On or about 06/03/2014.

29 CFR 1910.213(b)(7): Feeder attachment(s) did not have feed roll(s) or other moving parts so covered or guarded as to protect the operators from hazardous points:     a. In the pallet production area, an employee was exposed to an amputation hazard, in that, an ingoing nip point was created when the  gravity roller conveyor was installed beside the chain on the  single end power feed trim saw  # 2, on or about 06/03/2014.

29 CFR 1910.213(e)(1): Each circular resaw shall be guarded by a hood or shield of metal above the saw. This hood or shield shall be so designed as to guard against danger from flying splinters or broken saw teeth.    a. In the pallet recovery warehouse, employees were exposed to an amputation hazard, in that, the self adjusting guard on single end power feed trim saw  #1 was held open by a 3  inch piece of wood leaving the blade exposed during operation, on or about 06/06/2014.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):      a. In the pallet recovery shop, employees were exposed to electrocution and or fire hazard, in that,  cover for the 10HP 3 PH motor capacitor on the conveyor motor was partially knocked off leaving live wires exposed, on or about 06/03/2014.
