29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. WORKSITE: The employees painting the front side of the building from a 7/12 pitch roof were not provided with an appropriate fall protection system, exposing the employee to a 7 feet 9 inches fall to the ground.

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement. WORKSITE: The 6-foot fiberglass stepladder, used by an employee to paint a side of a building was placed on a 7 in 12 high pitched roof, and appropriately and safe means were not utilized to secure the ladder to prevent accidental displacement. On or about June 3, 2014, the stepladder slipped while the employee was painting, causing the employee to fall off the ladder and subsequently off the roof, sustaining serious injuries from the incident.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. WORKSITE: The employees, using ladders while painting a building were not provided with information and training associated with the type of ladder being used or the procedures necessary to eliminate or minimize those hazards.
