29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a) For employees working at 4139 North Washington Blvd., Sarasota, FL 34234 as observed on or about 07-02-2014 no training was conducted on the chemicals being used. Some of the chemicals that were used by the employees during the printing process were 3M Thinner CGS-15, HP Ink Cleaner, Klean-Strip Denatured Alcohol, Ink IP7-105, and 3 M Primer 94. Main ingredients to these chemicals included but were not limited to Ethanol, Xylene, Propanol, Cyclohexane, and N-Methylpyrrolidone.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the  safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  (a) For employees working at 4139 North Washington Blvd., Sarasota, FL 34234 as observed on or about 07-07-2014 no training was conducted on the new Safety Data Sheets and Pictograms. Some of the chemicals that were used by the employees during the printing process were 3M Thinner CGS-15, HP Ink Cleaner, Klean-Strip Denatured Alcohol, Ink IP7-105, and 3 M Primer 94. Main ingredients to these chemicals included but were not limited to Ethanol, Xylene, Propanol, Cyclohexane, and N-Methylpyrrolidone.

29 CFR 1910.305(g)(1)(ii): Flexible cords used as permitted in paragraphs (g)(1)(i)(C), (g)(1)(i)(F), or (g)(1)(i)(H) of this section were not equipped with attachment plugs:  (a) In the print shop by the EPSON ULTRA CHROME, PRO GS 600 printer employees were exposed to a shocked by hazard, in that, the electrical outlet box was repeled from the ceiling and not permenantly mounted.   (b) In the print shop by the ESPON ULTRA CHROME, PRO GS 600 printer employees were exposed to a shocked by hazard, in that, the electrical outlet box was placed on the floor without having been permanently attached to a fixed structure.
