29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut:  On or about June 4, 2014 at 1801 Sherwood Forest Blvd. Baton Rouge, Louisiana, the employer did not ensure a Ryobi table saw was equipped with a blade safety guard. 29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:  On or about June 4, 2014 at 1801 Sherwood Forest Blvd. Baton Rouge, Louisiana, the employer allowed employees to use an overturned 5 gallon bucket to gain height on the scaffold system. Employee was working from the third working platform at the time of the violation.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:  On or about June 4, 2014 at 1801 Sherwood Forest Blvd. Baton Rouge, Louisiana, the employer did not ensure that the scaffolding system was fully planked. Employees were working from scaffold platforms with gaps from 10" to 40" that were not planked.  Sandra & A. Stucco, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451.(b)(1), which was contained in OSHA inspection number 316289388, citation number 1, item number 2a and was affirmed as a final order on April 27, 2012, with respect to a workplace located at 29 Royal Palm Dr. Kenner, Louisiana.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.  On or about June 4, 2014 at 1801 Sherwood Forest Blvd. Baton Rouge, Louisiana, the employer did not ensure that ladder access was available to access scaffold platforms. Cross braces and framing were used as a means of access.  Sandra & A. Stucco, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1) which was contained in OSHA inspection number 316289388, citation number 1, item number 2b and was affirmed as a final order on April 27, 2012, with respect to a workplace located at 29 Royal Palm Dr. Kenner, Louisiana.

Stucco, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(h)(2)(ii), which was contained in OSHA inspection number 316289388, citation number 1, item number 1b and was affirmed as a final order on April 27, 2012, with respect to a workplace located at 29 Royal Palm Dr. Kenner, Louisiana.
