29 CFR 1910.213(d)(1): Circular handfed crosscut table saw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut: (a) At Billings & Associates Fine Woodworking LLC on or about June 4, 2014 the guard on the hood covering the saw blade above the Model DMLB-40 table saw was not completely enclosed. The see through cover that allowed the operator to view the cut being made by the saw was missing. (b) At Billings & Associates Fine Woodworking LLC on or about June 4, 2014 the guard on the hood covering the saw blade above the Model DMLB-40 table saw did not automatically adjust and remained raised from the table after the cut was completed.

The end of the cord was damaged and the outer electrical insulation was missing for approximately 10 inches on the cord.
