29 CFR 1910.106(e)(2)(iv)(d): Flammable liquids were not drawn from or transferred into vessels, containers, or portable tanks within a building only through a closed piping system, from safety cans, by means of a device drawing through the top, or from a container or portable tanks by gravity through an approved self-closing valve. On or about 3 June 2014, and at times prior thereto, employees would use the ball valve on a 55 gallon drum dispenser of Mineral Spirit 300 to fill smaller containers without the use of a self-closing valve, exposing the employees to a fire and explosion hazard.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. On or about 3 June 2014, and at times prior thereto, employees performing melting activities were wearing FR clothing arc rated at 8.7 for shirts and 23.8 for pants, exposing those employees to a burn hazard from splashing or popping of molten metal onto their clothing.

29 CFR 1910.133(a)(1): Protective face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment. On or about 3 June 2014, and at times prior thereto, two employees were working on a melting pot each in which at certain times, flux has to be added that may have rust on it, causing sparks to fly out during the melting process. During these times, the employees are wearing safety glasses but not a face shield or hard hats, exposing the workers to burns from being struck about the face and neck by hot sparks of molten metal.
