29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: (a) On or about June 2, 2014 at Die Cast Machine #4 an employee was burned on head, face, neck and arms when hand ladling molten aluminum from the dip well into the flux pan and was only wearing safety glasses, FR Coveralls and gloves.

29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use personal protective equipment: A) On or about June 2, 2014 at Die Cast Machine #4 an employee received burns to head, face, neck, arms when hand ladling aluminum from the dip well into a flux pan and had not received training on what personal protective equipment to wear, such as but not limited to, face shield, head protection and aluminized coat.
