CFR 1926.1408(b)(4)(ii)(A): The dedicated spotter was not equipped with a visual aid to assist in identifying the minimum clearance distance, i.e. a clearly visible line painted on the ground; a clearly visible line of stanchions and or a set of clearly visible line-of-sight landmarks. a) 380 Mauer Road, Track #5, Perth Amboy, NJ 08861. The crane operator came into contact with energized 26KV overhead power lines when he was not focused on the dedicated spotter for direction to maintain clearance of that energized overhead power line. Incident occurred on or about 06/03/14.

The crane operator came into contact with an energized 26KV overhead power line when the spotter was not using equipment, i.e. air horn or other device that would enable him to communicate directly with the operator. Incident occurred on or about 06/03/14.
