29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a). Worksite located at 735 NW 60th Street Unit A5, Kansas City, Missouri- Employees were working and  walking underneath elevated employees and materials which were falling to the ground in which the employees were not wearing any means of head protection.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a). Worksite located at 735 NW 60th Street Unit A5, Kansas City, Missouri- Employees were exposed to shock or electrocution hazards in that two separate radial saws had their cords severed, rewired and then had inner wires exposed to employees.

Worksite located at 735 NW 60th Street Unit A5, Kansas City, Missouri- Employee was observed working and walking on a suspended work platform and was not protected from falling by the use of any means of fall protection.
