Edison, NJ site - warehouse area: Industrial vehicle operators were not evaluated at least once every three years. The last documented forklift operator training was 5/24/2011. Condition noted on or about 6/05/2014. The Apparel Distribution, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA inspection number 313984833 (OIS #172632), citation number 01, item number 002 and was affirmed as a final order on 6/21/2011, with respect to a workplace located at 45 Saw Mill Pond Road, Edison, NJ 08817.
