29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.      At 18545 University Rd, Bldg 6, Sugar land, TX; employees worked on the third floor level at a wall opening unprotected at an approximate height of 22 feet, exposing employee to fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    At 18545 University Rd, Bldg 6, Sugar land, TX, employees used a makeshift wood ladder that was not secured or extended at least 3 feet to the upper landing to access the second floor at approximately 11 feet, exposing employees to fall hazard.
