29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about June 11, 2014 and times prior on machine GTM-4: The employer failed to ensure an employee followed established machine specific procedures and locked out the main power before going into the robot arm area to clear a miss-feed sheets which exposed him to the hazards of unexpected release of energy sources. b) On or about May 27, 2014 and times prior on machine GTM-4: The employer failed to ensure an employee followed established machine specific procedures and locked out the main power before entering the robot arm area to repair a mold which exposed him to the hazards of unexpected release of energy sources.
