29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  a) Jobsite located at 830 Southbridge Blvd, Savannah GA - On or about June 3, 2014, an employee was using an electric power blower to blow debris from the roof  area such as, but not limited to masonry debris with other employees in close proximity without the use of eye protection, exposing employees to eye injuries from ricocheting and flying debris.

The 9.5" wide wooden planks were secured and supported to the gable roofs to form a supported scaffold system, exposing employees to fall hazards up to 16 feet.
