Location: 60 Furman St., Brooklyn, NY. a) A tubular welded scaffold utilized by employees to erect bridge scaffolding was not fully planked. An employee fell approximately 12 ft. to the concrete floor below from the second level of the scaffold; on or about 6/5/14.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross-braces were used as a means of access. Location: 60 Furman St., Brooklyn, NY. a) Employees access the 2-level tubular welded scaffold by climbing the frames and cross braces while erecting the bridge scaffolding. Employees were exposed to fall hazards of up to approximately 12 ft. in height while climbing the scaffold's cross-braces to access the scaffold's platforms; on or about 6/5/14.
