29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Production area, 1980 Smith Township State Road, Burgettstown, PA 15021: Observed on or about June 02, 2014, energy control procedures were not utilized while maintenance work was performed on a Weldotron automatic wrapping machine (model number 1662, serial number FW53516). Machine operators were exposed to the hazards associated with the unexpected energizing or startup of the equipment while cleaning the seal bar, changing the paper, and removing jams from the seal head area of the machine. A safety interlock switch on the cover of the seal head area was not functional.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Production area, 1980 Smith Township State Road, Burgettstown, PA 15021: Observed on or about June 02, 2014, a Weldotron wrapping machine (model number 1662, serial number FW53516) was not used in accordance with instructions included in the listing and labeling. The wrapping machine was equipped with multiple safeguards which were disabled by the bypassing of the system controller. Inoperable safeguards included, but was not limited to, two emergency stop buttons and a safety interlock switch. Employees using the wrapping machine were exposed to electrical shock, burn, electrocution, cut-by, and amputation hazards.

The flexible cable entered the cabinet through an open door on the cabinet. The flexible cable was exposed to abrasive forces created by the movement of the electrical cabinet door. Employees were exposed to the hazards of electric shock while using the Weldotron wrapping machine.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: a) Production area (pizza room), 1980 Smith Township State Road, Burgettstown, PA 15021: Observed on or about June 02, 2014, employees used compressed air to clean floors and equipment. The air nozzles were not capable of reducing the air pressure to less than 30 pounds per square inch (p.s.i.). The air pressure was set to 140 pounds per square inch (p.s.i.) at the time of the inspection. Employees using the air gun to clean equipment and floors were exposed to the hazard of embolism. b) Pasta room, 1980 Smith Township State Road, Burgettstown, PA 15021: Observed on or about June 02, 2014, Observed on or about June 02, 2014, employees used compressed air to clean floors and equipment. The air nozzles were not capable of reducing the air pressure to less than 30 pounds per square inch (p.s.i.). The air pressure was set to 140 pounds per square inch (p.s.i.) at the time of the inspection. Employees using the air gun to clean equipment and floors were exposed to the hazard of embolism.
