29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form or scrap lumber with protruding nails was not kept clear from work areas:  The employer does not protect employees from tripping and puncture hazards when working on a construction site where form or scrap lumber with protruding nails was not kept clear from work areas. This violation was observed on or about June 4, 2014, on a construction site where employees were exposed to tripping and puncture hazards where debris with protruding nails was not kept clear from work areas.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that form or scrap lumber with protruding nails is kept clear from work areas.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     The employer does not ensure each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was protected by guardrail systems, safety net system, or personal fall arrest system. This violation was observed on or about June 4, 2014, at the construction site where an employee was exposed to falls greater than a 6 feet when performing roofing activities without a fall protection system.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees engaged in residential construction activities 6 feet or more above lower levels are protected from falling.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder(s) length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  The employer does not ensure that ladders used by employees are extended 3 feet above the upper landing surface. This violation was observed on or about June 4, 2014, at the construction site where employees were exposed to a 30 foot fall while using an extension ladder to access a roof without extending the ladder's side rails at least three feet above the roof landing.     Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that ladders used by employees have the ladder's side rails extended at least three feet above the roof landing.
