At the facility located at 6399 Shier Rings Road, Dublin, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses.  On or about May 12, 2014, an employee had a leg cut but there was no description of what caused the injury, what specific part of the leg was injured, and/or what the employee was doing when the injury occurred.  c.  At the facility located at 6399 Shier Rings Road, Dublin, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses.  On or about June 3, 2014, an employee had a back injury but there was no description of what caused the injury and/or what the employee was doing when the injury occurred.
