29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a.  At the work site, employees operated four fork lift trucks to move supplies and product throughout the plant and to load finished products onto trailers for delivery to customers.  The employer had not documented the certification of  training and evaluation of each fork lift truck operator as required by the standard.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  a. At the facility located at 6399 Shier Rings Road, Dublin, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses.  On or about March 12, 2014, an employee had debris in an eye, but there was no description of what caused the injury, which eye was affected, and/or what the employee was doing when the injury occurred.  b.  At the facility located at 6399 Shier Rings Road, Dublin, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses.  On or about May 12, 2014, an employee had a leg cut but there was no description of what caused the injury, what specific part of the leg was injured, and/or what the employee was doing when the injury occurred.  c.  At the facility located at 6399 Shier Rings Road, Dublin, Ohio, at the time of inspection, the employer had not recorded information in column (F) correctly for calendar year 2014 a description of work-related injuries and illnesses.  On or about June 3, 2014, an employee had a back injury but there was no description of what caused the injury and/or what the employee was doing when the injury occurred.
