29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: Platform P-44. The conveyor side of the platform did not have standard railings or equivalent to prevent employees from falling through the opening between the bottom of the conveyor housing and the platform. Employees were exposed to fractures and contusions in the event of a fall from the 9.5 foot high platform to the first floor level below. There is no abatement certification or documentation required for this item.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment was likely to be exposed to physical damage, enclosures or guards were not so arranged and of such strength as to prevent such damage: The cover for an electrical box containing energized 120 volt conductors was not firmly secured to the surface on which it was mounted, resulting in exposure to potential electrical hazards by employees working in the area when the cover was found apparently knocked off the box. There is no abatement documentation or certification required for this item.
