At the above addressed jobsite, on the roof, plywood used to cover the floor hole was not secured as to prevent accidental displacement, thereby exposing employees to falls. This most recently occurred on June 7, 2014, when an employee picked up a sheet of plywood, stepped forward and fell through the roof floor hole approximately thirty one (31) feet and seven (7) inches. Employee was pronounced deceased at the scene of the accident.
