A stairway, ladder, ramp or other safe means of egress was not located in trench excavations more than 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. On or about June 4th 2014 across from the Quick Trip at 1240 Daniels Bridge Road, Athens, Georgia: After excavating a trench approximately seven feet deep to install a thrust block and cap for a water line pressure test, an employee working in the trench did not have a ladder, ramp, stairway, or other safe means of egressing the trench.

On or about June 4th 2014 across from the Quick Trip at 1240 Daniels Bridge Road, Athens, Georgia: After excavating a trench approximately six feet deep to install a thrust block and cap for a water line pressure test, an employee working in the trench was not protected from cave-in.
