On or about June 4th 2014 across from the Quick Trip at 1240 Daniels Bridge Road, Athens, Georgia: After excavating a trench approximately seven feet deep to install a thrust block and cap for a water line pressure test, an employee working in the trench did not have a ladder, ramp, stairway, or other safe means of egressing the trench.
