29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and  the ladder side rails did not extend at least 3 feet (0.9 m) above the upper landing surface to which the ladder was used to gain access:    Employees were exposed to a fall hazard up to 9 feet when using a ladder to access the roof and the side rails did not extend at least 3 feet above the landing surface.

29 CFR 1926.20(b)(2): The employer did not provide frequent and regular inspections of the job site, materials, and equipment by a competent person:    Employees were exposed to a fall hazard while using a ladder to descend from the roof and a competent person had not performed frequent and regular safety inspections of the construction site to identify and eliminate the hazards.  Slater Construction & Siding Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 923507, citation number 1, item number 1b and was affirmed as a final order on 09/26/2013, with respect to a workplace located at 807 Jaime lane, Macomb Illinois, 61455.
