29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems or personal fall arrest systems:         On or about 06/05/2014during roofing activities, employee(s) installing asphalt shingles on an apartment building located at 5980 Marine Parkway in Mentor, OH without fall protection, were exposed to a fall hazard from a height of approximately 30 (+) feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards:         On and before 06/05/2014, at the address of 5980 Marine Parkway, Mentor, OH, the employer failed to ensure that employees completing roofing activities were trained in the appropriate use of fall protection. Equipment includes, but is not limited to, personal fall arrest systems or devices.
