29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems or personal fall arrest systems: On of about 06/05/2014 during roofing activities, the employer failed to ensure that employees completing the tear-off of old roofing materials from nine (9) apartment buildings located at 5980 Marine Parkway in Mentor, OH were protected from a fall hazard of 18(+) feet using a fall protection system or devices. A&D Contractors dba A&D Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926. 501(b) (13), which was contained in OSHA inspection number 313941411, citation number 01, item number 01(a) and was affirmed as a final order on March 09, 2010, with respect to a workplace located at 72 1021 S. Prospect St. Unit M, Hartville, OH 44236.
