29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position as they moved from the roof into the basket.  On or about June 6, 2014 at 330 U.S. Highway 78, Loganville, GA: Ten employees working 21 feet above the lower level installing metal decking were exposed to falling to the lower level as they climbed over the baskets guard rail system.

29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval. A basket was attached to the forks of an All-Terrain forklift to lift and lower employees to the working level.  On or about June 6, 2014 at 330 U.S. Highway 78, Loganville, GA: Where employees were using a basket to access and egress the roof, written approval from the forklift manufacturer was not obtained by the employer.

29 CFR 1926.752(b): Commencement of steel erection. A steel erection contractor shall not erect steel unless it has received written notification that the concrete in the footings, piers and walls or the mortar in the masonry piers and walls has attained, on the basis of an appropriate ASTM standard test method of field-cured samples, either 75 percent of the intended minimum compressive design strength or sufficient strength to support the loads imposed during steel erection.  On or about June 6, 2014 at 330 U.S. Highway 78, Loganville, GA: Where the steel erection contractor was erecting a pre-engineered building, erection of the structure was started prior to receiving written notification that the minimum compression design strength to support loads had been reached.
