29 CFR 1926.701(b): All protruding reinforcement steel, onto and into which employees can fall, was not guarded to eliminate the hazard of impalement:      a)   Five re-bar rods were not capped and sticking up two feet, located near the jobsite entrance steps. Employees were exposed to impalement hazard.       Location: 55 Pearl Street, Brooklyn N.Y.      On or about 5/14/2014      NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.1051(a):   A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided:      a)  During new construction a stairway was not provided at a 2ft ledge where construction personnel were going in and out of the building.  Unstable cinderblocks used as steps were a trip/ fall hazard.       Location: 55 Pearl Street, Brooklyn N.Y.
