Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a. On or about June 4, 2014, roof area, employees installing a TPO Roof System (rubber mat type) on a 1:12 pitch roof were not protected from falling 10 feet by the use of a guardrail system, safety net system, personal fall arrest system or any other type fall protection method.    The Southeast Contractors of North Florida, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 530099, citation number 01, item number 001 and was affirmed as a final order on September 11, 2012, with respect to a workplace located at 142 Cove Hollow Street, Lot 3, Santa Rosa Beach, Florida.    The Southeast Contractors of North Florida, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 313880593, citation number 02, item number 001 and was affirmed as a final order on February 14, 2011, with respect to a workplace located at 701 Harrison Ave, Panama City, Florida.
