29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked between the front uprights and the guardrail supports:      Employees were using makeshift scaffold platforms when applying cement skim coat to a 20 ft tall cinderblock wall:      a) Employee working from two planks on the scaffold which were uneven and can move apart, exposing employee to 10ft fall hazard.         b) Employee was standing on a single plank placed on top of an (orange) ladder being used as a bridge out to the scaffold.     Location: 55 Pearl Street, Brooklyn N.Y. 29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6m) above or below a point of access the employer did not provide one of the following; portable ladders, hook on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold.  Crossbraces shall not be used as a means of access:      a) Employees were exposed to fall hazard when climbing scaffold frames and crossbraces; employer was using several sets of pipe scaffolds to apply cement skim coat to the 20 ft tall cinderblock walls. Scaffolds were not fully planked, did not have a safe means to access and were not provided with guardrails. 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) though (vii) establish the types of fall protection to be provided to employees on each type of scaffold. Paragraph (g)(2) addresses fall protection for scaffold erectors and dismantlers:      a) Employees were exposed to fall hazard working on pipe frame scaffolds, adding a masonry skim coat to 20 ft high cinderblock walls. Scaffolds did not have guardrails, nor were personal fall arrest systems (PFAS, harness attached to lifeline) being used.      Location: 55 Pearl Street, Brooklyn N.Y.
