29 CFR 1926.501(b)(10):   Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:   a)  On June 9, 2014, at the address of 7001 Milan Avenue, Sandusky, OH: the employer did not assure employees were protected for fall hazards while installing roofing material on a commercial building.  The employees were exposed to an 18 foot fall to the ground below.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a)      On June 9, 2014, at the address of 7001 Milan Avenue, Sandusky, OH: the employer did not develop or maintain a written record of certification of fall protection training for all employees who might be exposed to fall hazards.
