OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:     a) On or about June 6, 2014, and at times prior, the employer exposed employees to struck-by and crush hazards in that employees were allowed to operate the Yale forklift without a seatbelt installed. 29 CFR 1910.23(a)(1): Every stairway floor opening was not guarded by a standard railing constructed in accordance with paragraph (e) of this section on all exposed sides (except at the entrance to the stairway):    (a) On or about June 6, 2014, and at times prior, the employer did not ensure that a stairway floor opening to a raised floor in the Mini service shop was guarded by a railing to prevent employees from falling to the level below.
