Maher/Elizabeth, NJ Site: Employee(s) do not wear provided Seat Belts exposing them to falls 1) Employee was not wearing the seat belt while operating Straddle Truck #4277 when he was catapulted from the cab, falling approximately 35 feet to the ground below. Observed on/or about 06/09/2014. Feasible and acceptable abatement measures among others are as follows: 1. Ensure that straddle truck operators wear seat belts when operating straddle truck machines.
