29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:    (a) The Plaza Hotel, Engineer Shop, basement of hotel.    On or about June 2, 2014,    Employees utilizing a Dayton Brand 10" Contractor Table Saw Model 4KXD1, were exposed to amputation hazards because the 10" saw blade was not guarded by an automatic adjusting hood to completely cover the portion of the blade above the table.     NOTE:  BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1910.215(a)(4): Employer did not ensure the Abrasive Wheel grinding machinery was not used with work rest(s) to support offhand grinding work:    (a) Engineer shop in basement of Plaza Hotel.    On or about June 5, 2014    Employees operating bench mounted abrasive wheel grinder and bench mounted wire wheel were not protected from caught in or struck against hazards as the grinder machines were not equipped with work rests to support off hand grinding work.    NOTE:  BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1910.215(b)(9): Employer did not ensure the guards for the  bench mounted abrasive wheel machine, where the operator stands in front of the machine, was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:  (a) Basement of Plaza Hotel, Engineer Shop.  On or about June 5, 2014   Employees utilizing the two bench mounted abrasive wheel grinder machines were not protected from caught in or struck by hazards because the manufacturer supplied guards to cover the abrasive wheels had been removed from the machines leaving the spindle nut and the abrasive wheels entirely exposed to the operator.   NOTE:  BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASE FILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION OR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    (a) Engineering department in basement of plaza hotel.    On or about June 5, 2014    Employees were exposed to electrical shock hazards because they were utilizing a power tap surge protector designed for light use, (Home Use) to energize a computer system, bench mounted power tools, and a portable air compressor. This use is inconsistent with the design and manufacture of the surge protector.     NOTE:  THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:    (a) The Plaza Hotel, Engineer Shop, Basement.    On or about June 5, 2014;    Employees working in the maintenance shop utilizing the work bench/counter were exposed to electric shock because the duplex outlet was not protected with the required cover/faceplate.
