Employees were not provided information and training on how improper fit, usage and maintenance could compromise the protective fit of the respirator.  An employee was observed wearing a full face respirator with a beard, which can interfere with valve function and fit. The employer did not ensure that each employee could demonstrate knowledge of why the respirator was necessary and how improper fit, usage, or maintenance could compromise the protective effect of the respirator.
