29 CFR 1910.176(c): Storage areas were not kept free from accumulation of materials that constituted hazards from tripping, fire, explosion or pest harborage:  a) Production Area: Eight plastic bags with Sodium Nitrite were stored on wooden pallet together creating a fire hazard, on or about 05/27/14 and 05/29/14.   b) Production Area: Twelve plastic bags with Sodium Nitrite were stored in about 3 feet from open drum with Para Amino Azo Benzene creating a fire hazard, on or about 08/06/14.

29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program required by 29 CFR 1910.1200:  a) Facility: Employees did not know about location of the company written hazard communication program at the workplace were employees worked with hazardous chemicals including, but not limited to Sodium Nitrite, Para Amino Azo Benzene, Meta-Toluenediamine, and Dextrine, on or about 05/27/14 and 05/29/14.
