Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold:  a. On or about June 05, 2014, at the above addressed jobsite, an employee performing bricklaying operations from a pallet on the forks of a powered industrial vehicle (Gradall 544B), approximately twenty-three (23) feet seven (7) inches above the ground, was exposed to injuries from falls due to the lack of guardrails.

On or about June 05, 2014, at the above addressed jobsite, employees working on the ground level entry way were exposed to ends of rebar that were not guarded, thereby exposing employees to the hazards associated with impalement.
