29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    Jobsite:    The employees working in the excavation were not wearing hard hats.   A cave-in of the trench wall occurred on or about June 09, 2014, in which an employee inside the trench was injured when his head struck a concrete electrical duct bank.

Jobsite:    The straight cut earth walls of a trench greater than six feet deep were not sloped, benched, or protected by a support or shield system exposing the employees to cave-in hazards.  On or about June 09, 2014 a cave-in occurred injuring one employee.
