29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation.  The employer does not ensure that scaffold legs bear on base plates and mud sills other adequate firm foundation.    a. This violation was observed on or about June 09, 2014 on the south side of the job site where employees were exposed to an approximate 15 foot fall hazard when working on an elevated pump jack scaffold without base plates and mud sills or other adequate firm foundation.    b. This violation was observed on or about June 09, 2014 on the east side of the job site between two residential buildings where employees was exposed to an approximate 30 foot fall hazard when working on an elevated pump jack scaffold without base plates and mud sills or other adequate firm foundation.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all scaffold legs bear on base plates and mud sills or other adequate firm foundation.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  The employer does not secure the scaffold poles to the structure at the bottom, top, and other points as necessary.    a. This violation was observed on or about June 09, 2014 on the south side of the job site where employees were exposed to an approximate 15 foot fall hazard when working on an elevated pump jack scaffold without the scaffold being secured at the bottom, top, and other points as necessary.   b. This violation was observed on or about June 09, 2014 on the east side of the job site between two residential buildings where employees was exposed to an approximate 30 foot fall hazard when working on an elevated pump jack scaffold without the scaffold being secured at the bottom, top, and other points as necessary.  Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure scaffold poles are secured to the structure by rigid triangular bracing at the bottom, top, and other points as necessary.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  The employer does not protect employees from falling when working on scaffold more than 10 feet above a lower level.    a. This violation was observed on or about June 9, 2014 on the south side of the residential building where employees were exposed to a fall hazard when working on an elevated pump jack scaffold at approximately 15 feet above the ground without being protected from falling to the ground.  b. This violation was observed on or about June 9, 2014 on the east side of the residential building where employees were exposed to a fall hazard when working on an elevated pump jack scaffold at approximately 30 feet above the ground without being protected from falling to the ground.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all employees working from a scaffold 10 feet or more above a lower level are protected from falling.
