29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a)  Worksite - Employee was not wearing a protective helmet and was exposed to being struck by asphalt shingles and tools while walking and working below overhead work; on or about 6/10/14.    Note:   Because abatement of this violation is already documented in the case file, the employer need not submit certification or documentation of abatement for this violation as normally required by CFR 1903.19.

29 CFR 1926.451(f)(6):   Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).  Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.   a)  Worksite - Employee was installing asphalt shingle on a two story residential home while working from a ladder jack scaffold.  The scaffold was set up less than 2ft away from an energized 220V power line; on or about 6/10/14.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
