29 CFR 1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections: A. Employees in heels area one were exposed to a Leeson grinder and the equipment was not provided with a guard that covered the spindle end and nut. B. Employees in heals area two were exposed to a Reliance Duty Master grinder and the equipment was not provided with a guard that covered the spindle end and nut. C. On the diabetic line, employees were exposed to five grinders (two Dayton grinders, two Leeson grinders, and one Gerson grinder) that were not provided with guards that covered the spindle ends and nuts.

29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: In the heels area two, employees were exposed to electrical shock hazard from a flexible cord leading to Dayton Grinder 22 and the outer insulation had pulled apart from the plug end and exposed the inner insulated wiring.
